Since 1986 our not-for-profit organization has been helping frail aged and disabled clients and their carers from around the Great Southern region to enjoy a better quality of life. We are continually working towards increasing our services to meet the changing needs of the community.
All prospective clients are assessed by a staff member to determine eligibility for services under the HACC guidelines and policy. Provision of service will be directed by client choice and need. Information regarding service options and fees will be discussed at initial assessment. Changes to client needs can be reviewed at any time. All information will remain confidential.
Albany Community Care Centre is dedicated to developing and maintaining the wellness of our clients. We are experienced in planning, delivering and supporting rehabilitation programs to help maintain our clients’ independence. Careful assessments help us determine how best we can help our clients remain living independently in the community. Respite care allows carers time out and the chance to develop social networks through our active Carer’s Group.
We offer services to support carers in their own homes and at our friendly Day Centre, and even Respite care for clients with low-level needs can also be provided in our Respite Centre, located adjacent to the Day Centre on Hardie Rd.
As well as regular activities, we six days per week at our friendly centre, our program includes a range of outings such as shopping, participating in community events such as the ANZAC services, Christmas lights tours, day trips to interesting places, picnics at the beach and even art group excursions to capture our beautiful environment at its best.
